Access to my Disclosure Statement

Just in case you need to review the fees, billing processes, missed appointment policies, or any other needs my disclosure is pasted below!

• My no show fee is my full session price at $120

• If you reach out to me within 48 hours after a missed session and still want to keep me as your provider, I am willing to reduce the fee to lessser price due to contact with me or one of my partnered teams at Mindful. 

  • This does not apply to Medicaid clients. 

Medicaid paitents are accepted but please discuss with my intake team at Mindful Therapy Group about specific your medicaid plans if it is not  listed on in network health insurance list. This may require you the client having to call your health insurance provider or case manager if known, to ensure that they will pay for your therapeutic services with me! 

If extra due dilliegence is required on your part to confirm payment by your health insurance please reach out to me as well! I can assist in ensuring you are connected to billing properly and help you understand your policy better.
